ABDOMINAL PARACENTESIS PROCEDURE DEFINITION:-

Introducing a needle into an abdominal cavity to collect the fluid
Or
Puncturing of the peritoneal (abdominal) cavity in order to remove of fluid(ascites)

Abdominal Paracentesis Purpose:-

  •  For relieving pressure in the peritoneal cavity,
  •  For drainage fluid from the abdominal cavity in Ascites condition.
  •  For pressure relief on the organs of the chest and abdomen,
  •  Cirrhosis of the liver with Ascites,
  •  For lab diagnosis (Histopathology e.g, cancer, Biochemistry values e.g. Albumin),

Equipment:-

  • Sterile tray
  •  Pigtail catheter,
  •  Sterile trocar and cannulla,
  •  Syringe 10ml,
  •  Needle Gauze No: 18,
  •  Three-way cannula,
  •  I.v. set,
  •  Sterile Test tube with a sealed container for lab diagnosis,
  •  Pint Jug (500ml),
  •  Vacuumed Collecting bottle or jar,
  •  Mackintosh and towel,
  •  Two sterile towels,
  •  Abdominal binder,

Surgical dressing set:-

  •  Spirit,
  •  Savlon,
  •  Betadine,
  •  Gauze pieces,
  •  Gauze pads,
  •  Transfer forceps,
  •  Cotton balls,
  •  Artery forceps,

Abdominal Paracentesis Procedure and Nursing Management:-

  •  Check for the physician’s order,
  •  Explain to the procedure to the patient and the patient relatives what to be going to done,
  • Take written consent from the patient and patient’s relatives,
  • Shave and Skin preparation should be done,
  • Record the patient’s vital e,g. Temperature,B.P, pulse,Spo2,
  •  Provide privacy,
  •  Maintain I. V, line, if any emergency to give fluids and medications,
  •  Paint the abdomen with Betadine,
  •  Assist to Doctor, giving needed articles, such as cotton swabs, sterile towels, etc…
  •  The needle should be inserted z-track technique,
  •  Monitor the patient during the procedure,
  •  Observe for fluid color,
  •  Measure fluid quantity,
  •  Send test tube for diagnostic tests,
  •  After finishing the procedure, seal the punctured wound with sterile dressing,
  •  Fasten the abdominal binder tightly, from the top to bottom,

Aftercare of the patient:-

  •  Provide any hot tea if indicated,
  • Monitor patient vitals continuously,
  • monitor input and output chart,
  • Watch for any reaction for 24 hours,

Abdomin Paracentesis Complications:-

  •  Hypo-tension,
  •  Heart failure,
  • •Internal bleeding,
